This is a piece of Doctor Who fan art using the recognizable person outline (as seen in places such as outside restrooms), and cleverly placing the according type of heart inside of the optimist, pessimist, realist, and "me" the Timelord (aka the Doctor, who has two hearts). It effectively, and with underlying humor, represents each of the hearts the types of people would contain.
